Registering with the Practice
To register with the Practice you will need to collect and complete all required documentation for each family member within the Registration Pack. Once the forms have been completed, please return them to the Surgery along with 2 forms of identification; one has to have a photograph (passport, driving licence) and one proof of address. You will then be registered with the Practice and be allocated a named Doctor.
You will be asked to make an appointment for a New Patient Health Check, these are free for all new patients. The Health Care Assistant will discuss your general health, and check your blood pressure, weight and urine sample.
Your medical records will be requested and sent to the surgery in due course.
There is a detailed map of our practice boundary area at the surgery and we are only able to accept new registrations within our practice boundary area. This map shows the approximate area covered by the practice.
