West Street Surgery is registered and complies with the Data Protection Act 2018 (DPA 2018).

Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. If you wish to obtain a copy of your medical records, please complete the Subject Access Report (SAR) form. Once you have completed the form, we will prepare your medical records for you to collect from our surgery. You will need to provide some form of ID when collecting your medical records.

We are aware that patients will sometimes require access to their medical records for the purposes of legal claims. If you require your medical records for solicitor or insurance claims we will require patient consent before we release any information.
