Accessing someone else’s information

Family members or other individuals who wish to contact West Street Surgery regarding a patient’s care or personal information must have the patient’s written consent before any information can be disclosed.

The written consent will be uploaded to the patient’s record and appropriately coded to ensure that there is a clear record of the patient’s consent. We must stress this is written consent an not verbal consent.

In the absence of written consent, staff will be unable to provide patient information or discuss the patient’s care with family members or other third parties. This is to ensure that patient confidentiality, privacy, and information governance requirements are maintained at all times.

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us and it means that you will be able to see both records. This is generally used for linking a child’s record to an adult.

To request proxy access: collect a proxy access form from reception during opening times

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.
